Permission for Whitireia and WelTec to capture imagery and/or video and share in the manner set out in this form.

If you're a student, please fill out the Sharepoint version of the form. Keep in mind you will need to log into your Microsoft account. If for some reason you can't login to your account, please use the PDF form instead and follow the associated instructions.

If you're not a student, please complete this fillable PDF and return it to talentrelease@wandw.ac.nz, alongside an image of yourself. The image will only be used to identify you in the associated imagery or video.
